Semaglutide vs Tirzepatide: What the Head-to-Head Trial Showed, and What Each Costs in Waukesha

Medical weight loss consultation at Raw Glo Med Spa in Waukesha, WI

Almost everyone who comes in asking about weight loss injections has already heard both names, usually from someone who had a good result on one of them. The question underneath is always the same: is the more expensive one actually better, or is that just what it costs. There is now one trial that compared them directly, head to head, at full dose, so the answer is less speculative than it was two years ago. Here is what it found, what each one costs per month here, and the parts people tend to find out later.

The short version

  • Tirzepatide lost more: In the only head-to-head trial, tirzepatide produced 20.2 percent average weight loss against 13.7 percent for semaglutide over 72 weeks. That gap is real and it is large.
  • Semaglutide costs less: Here it runs $250 to $375 a month depending on dose. Tirzepatide runs $380 to $540. Over a year that difference is roughly $1,500 to $2,000.
  • Neither is a prescription you collect and leave: Both need dose titration and monthly check-ins, which is most of what separates a supervised program from an online one.
  • The weight comes back if you stop and change nothing: That is true of both, and it is the single most important thing to understand before you start.

What each one actually is

Semaglutide is a GLP-1 receptor agonist. GLP-1 is a hormone your gut already releases when you eat; it slows how fast your stomach empties and signals fullness to your brain. The drug mimics it, for far longer than your own version lasts. The practical effect is that you feel full sooner and stay full longer, and the constant background noise about food gets quieter.

Tirzepatide does that and one more thing. It acts on GIP receptors as well, a second gut hormone involved in how your body handles insulin and fat storage. That is why it gets called a dual agonist, and the working theory for why it outperforms is that two levers move more than one. Whether the GIP arm matters because of appetite, metabolism, or something not yet pinned down is still being argued in the literature.

Both are once-weekly injections you give yourself, in the stomach, thigh, or upper arm, with a very short needle. Neither hurts the way people brace for.

The only head-to-head trial, and what it found

SURMOUNT-5 is the trial worth knowing about, because almost everything else comparing these two is an indirect comparison across separate studies with different participants. This one enrolled 751 adults with obesity and without diabetes, put them on the maximum dose each could tolerate, and ran for 72 weeks. Results were published in the New England Journal of Medicine in May 2025.

Tirzepatide produced 20.2 percent average weight loss. Semaglutide produced 13.7 percent. The estimated difference was 6.5 percentage points, and it was statistically significant. Waist circumference fell 18.4 cm against 13.0 cm. Close to half the tirzepatide group lost at least a fifth of their body weight, against 27.3 percent on semaglutide.

What each costs at Raw Glo

Both are priced by dose rather than as a flat program fee, which means your monthly cost rises as you titrate up and then settles once you find the dose that works.

Monthly cost by dose tier
Dose tier Semaglutide Tirzepatide
Low $250 $380
Mid $330 $510
High $375 $540

At the high tier the gap is $165 a month, close to $2,000 across a year. That is the number to hold next to the 6.5 point difference in the trial, because the honest version of this decision is a trade rather than a right answer. Full pricing and booking sit on the medical weight loss page.

The side effects, honestly

Nausea, constipation, reflux and fatigue are the common ones for both drugs, and they are worst in the first weeks after a dose increase. This is the single best argument for titrating slowly rather than chasing the top dose: most people who quit these medications quit during a jump they did not need to make that fast.

The serious risks are real but uncommon, and they are a conversation for a consult with your history in front of us rather than a paragraph on a website. A personal or family history of medullary thyroid carcinoma or MEN2 rules both drugs out, as does a history of pancreatitis, and neither is used in pregnancy.

Which one suits you

  • Semaglutide, if cost is a live constraint: Thirteen percent of body weight is not a consolation prize. If the $2,000 a year difference would make you stop after six months, the cheaper drug you stay on beats the better drug you abandon.
  • Tirzepatide, if you have a lot to lose: The further you have to go, the more those 6.5 points compound into something that changes your health rather than your jeans.
  • Semaglutide, if you are nervous about side effects: Not because it is gentler by design, but because starting lower and cheaper makes it easier to find out how your body responds without a large financial commitment riding on it.
  • Either, if you will actually come to the check-ins: The program is the supervision. Neither drug does its job well when the dose is never adjusted.

What happens when you stop

Appetite returns. Without changes to how you eat and move, the weight tends to come back, and that is documented for both drugs rather than being a thing clinics say to keep you enrolled. It is the reason our program is built around monthly follow-ups instead of a prescription and a handshake, and the reason the conversation at your first visit is about what the next two years look like rather than the next eight weeks.

If you want to understand how these sit alongside the other options we offer, IV therapy, vitamin injections, peptides and weight loss support covers where each one actually fits.

Booking a consult

A consult is a licensed provider going through your health history, your current medications and what you are trying to reach, then telling you whether either drug is appropriate. If you are not a candidate we will say so. You can book on the medical weight loss page or call us and talk it through first.

Frequently asked questions

Can I switch from semaglutide to tirzepatide?

Yes, and people do, usually when weight loss stalls at a dose that is already near the top. There is no washout period, but we do not match the dose across one to one because they are different drugs on different scales. You restart the titration lower on the new one and build up again.

How soon will I notice anything?

Appetite usually changes within the first week or two, often before the scale moves at all. Visible weight loss typically takes four to eight weeks to become obvious, and the trial numbers above are measured at 72 weeks, which is worth remembering when month two feels slow.

Do I have to inject myself?

Yes, once a week, and we show you how at your first visit. The needle is very short and goes into fat rather than muscle, so it is far less dramatic than people expect. Most clients say the anticipation was worse than the first injection.

What if I miss a dose?

Take it when you remember if it is within a few days of your usual day, then go back to your normal schedule. If it has been longer than that, skip it and resume. Do not double up, because the side effects scale with the dose and a double dose is how people end up having a miserable week.

Is this the same as Ozempic or Wegovy?

Those are brand names for semaglutide, and Mounjaro and Zepbound are brand names for tirzepatide. Medications prescribed in a med spa setting are often compounded by a pharmacy rather than dispensed as the branded product, which is a different regulatory category at a very different price. Ask us which you are being prescribed, and we will tell you plainly.

Will insurance cover it?

Usually not for weight loss in this setting, which is part of why the monthly pricing is listed openly above. Coverage for these drugs is inconsistent and depends on your plan and whether there is a diagnosis attached, so check with your plan administrator rather than assuming either way.
